The diaphragm is the main muscle of respiration. When it contracts it moves downwards and increases the vertical diameter of the thorax. In respiration it works reciprocally with the muscles of the anterior abdominal wall. When the diaphragm descends, the abdominal muscles are pushed forwards. When the diaphragm ascends the abdominal muscles move backwards. These movements are considered in more detail later (p. 19). The diaphragm is also involved in activities in which increased abdominal pressure occurs due to contraction of the anterior abdominal muscles.

The Diaphragmatic Openings. There are three main openings in the diaphragm (figure 17). The aortic opening transmits the descending thoracic aorta from the thorax to the abdomen where it becomes the abdominal aorta. This opening actually lies behind the diaphragm in the midline at the level of the twelfth thoracic vertebra. The thoracic duct and the beginning of the azygos vein on the right of the aorta pass upwards through this opening. The oesophageal opening is about 2 em to the left of and anterior to the aortic opening.
