It is normal for children to respond to trauma with distress. However once the danger has passed such children should slowly return to normal. In PTSD the symptoms persist with intrusive images of the event as if they are re-experiencing it, or there is marked avoidance behaviour or significant physiological arousal. Much of the literature is based on adults but it is likely that there will be more descriptions of PTSD in childhood in the near future. Epidemiology Many studies have shown that PTSD occurs in children from diverse ethnic and cultural backgrounds though the presentation may differ.

The term school phobia is often used synonymously with school refusal but should be reserved for those situations where the feared element is specifically within the school. Both terms need to be distinguished from truancy (see chapter 5) in which there is no anxiety about school attendance, but rather the child chooses to stay away from school, usually under peer group influence.
